Radiologic Assessment of Cement Mantle Thickness of the Femoral Stem

Abstract
|
|
|
Purpose: The purpose of this study is to confirm the accuracy of cement mantle thickness through postoperative simple lateral radiograph, in patients treated with cemented hip arthroplasty.
Materials and Method: We analyzed radiologically 18 patients treated with cemented hip arthroplasty. Mean age was 71.9 years(51 to 84). Two types of cemented femoral stem were used in this study: MS30 femoral stem (PROTEK,
SULZERMEDICA®) and EXTER femoral stem (Howmedica®). Hip joint true lateral views and various rotational views were taken such as external rotation 20°, 10° view, and internal rotation 10°, 20° view, form true lateral film. On hip joint true lateral view, cement mantle thickness appeared to be minimum in mostly distal portion of the Gruen zone Ⅷ and proximal portion of zone Ⅸ. So that we analysed in this zone, and then ANOVA test was practiced.
Results: MS 30 stem, the mean cement mantle thickness 2.11 ㎜, 2.94 ㎜, 2.59 ㎜, 1.82 ㎜, 1.73 ㎜ in true lateral, external rotation 20°, 10° views, and internal rotation 10°, 20° views, respectively(P<0.01). Exter stem, the thickness
1.88 ㎜, 1.74 ㎜, 1.69 ㎜, 1.67 ㎜, 1.60 ㎜ in true lateral, external rotation 20°, 10° views, and internal rotation 10°, 20° views, respectively(P=0.632). We could realize that cement mantle thickness appeared to be minimum in internal rotation
20° view.
Conclusion: In case of proximal trapezoid tapered straight cemented femoral stems, postoperative cement mantle thickness is measured differently according to the different rotation angles of the lateral x-ray. Therefore this study suggests that there is needed to confirm the thickness of local cement mantle around the femoral stem by taking the x-rays at simple true lateral radiograph and its internal rotation 20° view.
